If your patient’s Annual Wellness Check has identified them as having two chronic conditions, we will register that person as eligible for Chronic Care Management (CCM). CCM has been proven to reduce hospitalizations by 5.6%* and ER visits by 4.2%. As an added bonus, these improved outcomes will help increase and maintain your MIPS score.
RPS will contact patients on your behalf via a personal one-on-one phone call, not an impersonal email or robo call. We explain the program to your patient, schedule and administer their monthly appointments, and notify you of appropriate billing codes.
SCHEDULE: We will contact your patients on your behalf to explain the program and set up times for CCM to begin or continue.
ADMINISTER: Our medical staff will treat your patients with the same level of care and compassion to which they are accustomed.
REPORTING: Your staff will receive reporting with appropriate billing codes ready for submission
